This is to certify that Mr./Miss... was honored University Social Work Award due to his/her excellent performance in student activities of the academic year from 20.. to 20....

2. (month), (year)
This is to certify that Mr./Miss... was honored the Second Prize of University Scholarship due to his/her hard exerti##被过滤## and outstanding achievement of the academic year from 20.. to 20....

3.(month), (year)
This is to certify that Mr./Miss... was awarded the Honor of Excellent Youth League Member.

最主要的是时态的错误。
