　　It is my pleasure to enclose the completed application forms and the required materials as list below:

1. The application form - completed

2. The application fee - US$60

3. Statement of Purpose

4. Two transcripts of college record from National Taiwan University -- enclosed

5. The certification of the scholarships

6. The certification of the high school program

7. The name and address lables - completed

8. Indification of need for finacial support

9. Xerox copies of the transcripts of GRE(general),TOEFL and TSE

10. A Xerox copy of the admission ticket of GRE(advanced)taken on XXXX(date)

11. A copy of the term project on 2-D Ising model

12. Personal Resume

13. Letter of Recommendation:
　　Prof. Chong Der Hu's letter is enclosed.
　　Prof. Shin Nan Yang's and Prof. Mong Na Lo's will be sent directlly soon.

14. Report of English Proficiency will be sent directly soon.I have made the request for ETS to send the transcripts of TOEFL and TSE. The GRE transcript will be sent after it is applicable. P.S:

　　Cover letter is the highlight of resume. It must be short and emphasize in some outstanding point of your exprience. Remember, the function of cover letter is only to make the people who read your letter have interest to read your resume.But few U.required Cover Letter, maybe Sloan B-Scool of MIT require it. If you write cover letter to B-School, especially top-20 B-school, you must write it according to standard business form。

我申请大学从来没用过cover letter, 也没见失败。一来学校没要求，二来觉得没必要提供，因为重要信息都分散在其他材料里清晰表达了。觉得一份清楚的材料清单附在最前面比这个要重要的多。
