Purpose of the Study:  I understand I will be learning about a video game.  I know that the researchers are studying how people learn in highly interactive environments.  I realize that in the experiment I will answer basic questi##被过滤## about the basic concept in the game, and will then practice the basic commands on the controller keyboard.   Finally, I will play the actual game for about a half hour.  I am aware that I will be videotaped during the experiment so that the researchers can find out how I learn to play the game.

I understand that the following procedure will be used to maintain my anonymity in analysis and publication/presentation of any results.  Each participant will be assigned a number, names will not be recorded.  The researchers will save the data and videotape files by participant number, not by name.  Only members of the research group will view the tapes in detail.  The videotapes and records will be stored in locked files by Professor _____________.  No other researchers will have access to these files.

Sample Faculty Reference Letter

Dear [Name of Employer]:

This reference letter is provided at the written request of [name of student], who has asked me to serve as a reference on [his/her] behalf. It is my understanding that [name of student] is being c##被过滤##idered by your organization for the position of [job title]. Please be advised that the information contained in this letter is confidential and should be treated as such. The information should not be disclosed to [name of student, if student has waived access] or anyone in your organization who would not be involved in the hiring decision regarding this individual. Additionally, the information should not be disclosed to anyone outside of your organization without the c##被过滤##ent of the student.

I have known [name of student] for the past [number of months, semesters, years] as [he/she] has taken the following courses which I teach: [list courses, give brief description of content of course]. As [his/her] professor, I have had an opportunity to observe the student's participation and interaction in class and to evaluate the student's knowledge of the subject matter. Reference Checking Sample Letter

As your c##被过滤##tituent, a human resource professional, and one of the nearly 2,000 Nebraska members of the Society for Human Resource Management (SHRM) or its local affiliates, I want to take this opportunity to express my support for LB 378, which would allow employers to give good faith job reference information without facing civil liability.  

LB 378 ensures that employers in Nebraska have access to vital information about prospective employees. This legislation provides employers with immunity from civil liability when providing job reference information in good faith, restores an employer’s ability to recruit and hire the most qualified workforce, and re-establishes the rights of good employees to have their performance c##被过滤##idered in the hiring process.  In addition, the bill protects the rights of employees by providing an opportunity to legally challenge the presumption of good faith.  

In the past decades, a handful of highly publicized defamation cases stemming from the hiring process have caused employers to adopt nondisclosure policies to prevent liability.  As a result, employers across the state are unable to obtain reliable and accurate job performance information on prospective employees.  This situation impedes an employer’s ability to hire qualified employees and opens employers to the prospect of hiring inappropriate or dangerous individuals.  Due to liability concerns, employers who implement nondisclosure policies deny good employees the right to have their performance c##被过滤##idered in the hiring process, and hide the history of bad employees with inappropriate behavior and/or substandard performance.  Therefore, it is not only the employer who suffers, but the employee as well.

As employers, we need more information to aid us in hiring decisi##被过滤##.  We have to protect our current employees and customers from violence, threats, harassment and other instances that cause harm while ensuring we hire the most qualified candidates.  For these reas##被过滤##, 35 states have passed reference checking legislation, and the issue is under debate in several other states.  

Please ensure Nebraska joins the ranks of states that provide reasonable hiring procedures that protect both employers and employees.  I urge you to support LB 378.

SAMPLE TERMS OF REFERENCE
RWSS INTERVENTION DESIGN FOR:
(NAME OF MULTI-SECTOR PROJECT)
(NAME OF COUNTRY)


BACKGROUND

(Provide brief description of the project, including its background, stage of development or implementation, geographic scope, etc., drawing on country assessment or other preliminary information on the RWSS sector).

OBJECTIVE

The objectives of the RWSS component design are to establish an implementation strategy, define project activities and prepare guidelines under which RWSS sub-projects will be funded by the (name of multi-sector project).  

(Add any additional information or objectives identified in the country assessment.)

TASKS

The assignment will be divided into the following tasks:

Task 1: Summarize Policy and Institutional Issues  

Based on the country assessment or other existing information, summarize the main policy and institutional issues for RWSS, with particular reference to sustainable service delivery, to be financed under the project.  This would include, but not be limited to:

•        Policy and institutional environment and extent to which the demand-resp[&&&&&@@@@@]ive approach is used.
•        Extent of unmet demand for RWSS services and performance of other projects or programs.

Task 2: Carry Out Sector Dialogue and Participatory Strategy Formulation  

In view of the overall policy and institutional issues summarized above, initiate a participatory process with the major stakeholders and arrive at a c[&&&&&@@@@@]ensus on the basic strategy for including RWSS components in the project.  This would be implemented through a series of c[&&&&&@@@@@]ultati[&&&&&@@@@@]/workshops, and would address how and under what conditi[&&&&&@@@@@], RWSS investments will be made under the project.  It would include, at a minimum, the following strategic themes:

•        Policy and institutional framework. Are fundamental reforms needed to the existing policy and institutional framework, before RWS can be c[&&&&&@@@@@]idered part of the project?  For example, reform of the central or sector-based agency.  If so, how will they be addressed?  Within the project? Or in a sector-based project?

•        Roles and resp[&&&&&@@@@@]ibilities.  Clarify areas of resp[&&&&&@@@@@]ibility and collaboration for RWSS components of the project. For example, should RWSS subprojects be required to collaborate with existing sector-based instituti[&&&&&@@@@@], and/or existing norms? Do such norms need be created?

•        Establishing technical norms and making service level opti[&&&&&@@@@@] available to communities.  Recommend a policy and approach to ensure availability of service level choices to communities and adherence to technical norms during both the investment and operational phases. This should include a policy and implementation framework for communicati[&&&&&@@@@@] with the community at all stages of the subproject cycle.

•        Hygiene and sanitation issues.  Recommend a policy and approach to ensure that hygiene and sanitation issues become part of the RWSS component.

•        Finance and cost recovery.  Recommend a policy and approach for finance and cost recovery for both the investment and the operational phases.

•        Community support and institution-building.  Define the strategy to ensure local capacity for sustainable service provision.  This will include:

-Local g***** capacity to backstop communities

-Support to NGOs and private sector to provide sustainable supply chains for software and hardware components

-Community contracting mechanisms

-Ensuring environmental and social safeguards and mitigation during installation and operational phases

Task 3: Define RWSS Component and Activities

Based on the results of Task 2, specify the types and estimated magnitude of RWSS investments that would be financed under the project.  If applicable, define sector-specific institutional and capacity-building activities to be financed under the project. Describe how and where linkages with other subprojects or programs (such as health-related projects) will be promoted.
